Providers familiar with the latest technology in regenerative aesthetics know about the power of exosomes. These extracellular vesicles are popular in skin rejuvenation and are proving to be a powerful tool to complement aesthetic treatments including radiofrequency microneedling (RFMN) and skin resurfacing. Today, plant-derived exosomes are also emerging as an effective and safer option for advanced skincare and hair technologies.

“Exosomes are extracellular vesicles secreted by nearly all cell types, including stem cells, that play a crucial role in cell-to-cell communication,” explained Beverly Hills, Calif.-based dermatologist Nathan Newman, MD. “They contain bioactive molecules that they transfer to other cells to influence growth, repair processes and tissue regeneration. Plant-derived exosomes have the ability to influence human cells through their bioactive substances, which have antioxidant, anti-inflammatory and regenerative properties.”

Created by Dr. Newman, EXO|E™ Skin Revitalizing Complex from Aesthetic Management Partners (Cordova, Tenn.) is an advanced biometric formula that uses plant stem cell-derived secretory factors in a three-step system that is chemical-free, water-based and made in the U.S. The treatment protocol includes D|TOX, EXO|E and RE|PAIR used both in the office and at home by patients to continue treatment, enhance results and expedite desired outcomes. DE|RIVE™ Hair Wellness System is a natural plant-based system also powered by EXO|E technology for comprehensive hair and scalp health.

Natural Rejuvenation Benefits

According to Dr. Newman, secretory factors from both human and plant cells have the ability to influence human cells. “The EXO|E [Skin Revitalizing] Complex is a natural, topical skincare system designed to enhance cellular communication and skin regeneration and contains concentrated bioactive molecules such as exosomes, growth factors, liposomes and anti-inflammatory factors that are not processed or adulterated,” he explained. “It promotes healthier, more youthful-looking skin and is especially effective in post-procedure recovery. Although the product is water based, it feels silky and moisturizing on the skin. This system has become part of all my treatments, from fillers and neurotoxin to lasers, RFMN and surgical procedures.” Notable benefits include enhanced skin smoothness, firmness and hydration, as well as improved long-term outcomes in skin rejuvenation protocols, he added.

“We commonly use the EXO|E, D|TOX and RE|PAIR products with ablative lasers and RFMN treatments,” reported David Goldberg, MD, JD, director of cosmetic dermatology and clinical research at Schweiger Dermatology Group (New York, N.Y.). The benefits he sees are quicker healing and greater patient satisfaction, while his patients report they enjoy the soothing effects.

A Safe, Effective Regenerative Option

According to board-certified dermatologist Kristen Aguirre, MD (Oceanside, Calif.), since plant-based exosomes are not derived from human cells, they do not carry the same risks of infectious contamination, genetic material transfer or ethical sourcing controversy. “The EXO|E Complex is a blend of plant-derived exosome-like nanoparticles,” stated Dr. Aguirre. “These are sourced from plants that produce the desired signals to rapidly and effectively reduce inflammation, increase the production of hyaluronic acid, regulate pigment production and even support hair growth.

“I see a rapid improvement in procedure-related erythema, discomfort and heat during and immediately after the procedure, as well as improved recovery, improvement in both speed and quality of results, and patient satisfaction as they feel engaged and well cared for in the process,” Dr. Aguirre added. “My patients appreciate that I am taking the extra steps to enhance their experience and results, which equates to a higher level of care and value for their investment.”

When it comes to procedural recovery and results, Dr. Aguirre hears terms from her patients such as “game changer” and “luxurious,” she reported. They also notice the improved effectiveness of topical numbing when using this system. “They now view the numbing experience as an enjoyable part of the process, as the silky texture soothes them prior to the numbing application, and they report less burning and discomfort during procedures.”

A Game Changer for Hair Health

“DE|RIVE is a system of plant-based exosomes targeted at calming inflammation, balancing skin oils and stimulating winglessrelated integration site (Wnt) pathways and cytokeratin-15 expression, two integral pathways in healthy hair growth,” Dr. Aguirre continued. “This system includes a potent in-office serum designed to rapidly penetrate the scalp. I see a remarkable improvement in my patients’ scalp health, including less redness, flaking, itchiness and sensitivity.” Her patients also take home DE|RIVE daily serum and the companion shampoo and conditioner to continue full scalp support.

According to Dr. Newman, DE|RIVE is not irritating or greasy and his patients see improvement soon after implementing the system. “They report seeing improvement in their hair within the first month of beginning the treatment,” he shared. “DE|RIVE is a game changer and the improvement I see in women with hair loss is life changing.”

Dr. Goldberg also uses the DE|RIVE hair treatment system for patients who desire improved hair growth and conducted his own in-office study. “DE|RIVE represents plant-based exosome-like packets that have been shown in laboratory settings to decrease inflammation, increase the growth of new blood vessels and promote differentiation of human stem cells toward hair,” he reported. “Our study aimed to look at real life human data in both men and women with hair loss. We applied the DE|RIVE products to the scalp either before ultrasound treatment or after RFMN for three treatments, one month apart. In both scenarios we saw increased hair growth about one month after the third treatment.”

The EXO|E skin system and DE|RIVE hair system are novel approaches for providers to offer their patients the latest in regenerative aesthetics. “My patients really love that these are natural, plant-derived products, as here in Southern Calif. there is a strong trend toward natural products and away from pharmaceuticals and surgery,” added Dr. Aguirre. “Patients who have experienced procedures without the use of the EXO|E system freely tell me how much faster and easier the recovery is with the EXO|E system and, going forward, they never want to have procedures done without them.”
