“Discovery PICO features true wavelengths for all three: the 1064 nm, 532 nm and ruby 694 nm,” said dermatologist Jordana Herschthal, MD, of The Herschthal Practice Aesthetic Dermatology (Boca Raton, Fla.). “Because of its utility I have actually had recent growth in tattoo removal, which is something I really didn’t push for. Most often I harness it for patients with skin aging signs and especially in combination with other modalities for acne scars. If I have to use a laser for deep benign pigmentation, this is my go-to.” In addition to pico- and Q-switched nanosecond pulses, OptiPulse and PhotoThermal proprietary pulse structures provide longer pulses. Together, these four emission modes are termed Quattropulse Technology. The device’s OptiBeam II technology provides a homogeneous beam profile. Fractional and full-spot delivery are dependent on the choice of handpiece.

Dr. Herschthal believes the device’s global utility comes from its adjustability. “Between the available wavelengths, pulse structures, peak power and modulating basic settings, you can treat any patient with this device,” she explained. “You cannot treat everything every patient presents with, but overall, nobody is untreatable. I treat a variety of skin types as well as both young and old patients confidently, so it is a true workhorse in my practice. Brown spots and benign vascular issues, wrinkles and acne scars, skin resurfacing and more, the device is comprehensive in its capabilities and eminently customizable.”

Dermatologist Celina Ying Yen Chuang, MD, of the DRX Dermatology Clinic (Taipei, Taiwan) credits the high peak power and low pulse duration for Discovery PICO’s ability to treat with minimal risk of post- inflammatory hyperpigmentation (PIH). “It also has an array of handpieces and spot sizes, which means minimal limitations for physicians utilizing this device. For example, I can do a generalized pico toning procedure for benign pigmentations on the entire face relatively quickly using a very large spot size. And, because of the high peak power, the amount of energy delivered with the large spot size is still sufficient for good results. I can also do a delicate job treating small areas of pigmentation around the eyes using a smaller spot size.”

Dr. Chuang uses Discovery PICO for many things, but regularly for acne scarring and skin resurfacing. “The high peak power allows the device to cause optical breakdown of even hard scar tissue, which is particularly useful for improving surface irregularities such as acne scars. The rapid plasma formation seen with picosecond lasers causes an ablative effect without PIH, which is otherwise quite common when using ablative lasers to treat Asian skin.”
