Patients are demanding more from their aesthetic providers and the treatments they offer. Many patients are requesting to look more like their favorite app filters with even-toned, youthful skin, or in some cases, snatched like their favorite celebrities. Desiring these results is one thing, but wanting them with little to no downtime or pain is another.

Having only fillers or neuromodulators in your treatment arsenal is a thing of the past. Today’s aesthetic physicians know that to improve patient outcomes and practice success, they need to include the most advanced regenerative aesthetic therapies that can be combined with other treatments, and for good reason: 82% of patients surveyed stated they left aesthetic practices that did not offer more innovative treatment solutions that met the standard of little to no downtime or pain. An additional 8% said the inability of practices to treat their skin color safely and effectively was also an issue.1

With today’s patients being bothered by lines, wrinkles, skin texture and discoloration, skin revitalization and dermal remodeling comprises 70% of market demand. Approximately 56% of consumers are considering a treatment that will help smooth wrinkles and tighten skin; 90% of pregnant women, 70% of adolescent females, and 40% of adolescent males are struggling with stretch marks; another 40% are dealing with acne or acne scars.1

“I don’t sell individual products or treatments, I sell outcomes using the best-in-class regenerative trifecta from BENEV,” explained Suneel Chilukuri, MD, director of cosmetic surgery at Refresh Dermatology (Houston, Texas). “This includes Sylfirm X, a cutting-edge radiofrequency (RF) microneedling device for non-invasive skin revitalization, which features the proprietary FDA-registered pulse wave (PW) and continuous wave (CW) RF that targets both skin laxity and pigmentation for ultimate skin rejuvenation. I also use Exosomes by BENEV, a scientific biotech breakthrough for skin and scalp regeneration, and the company’s MIRACU PDO line of threads which provides a large variety of thread products for customized patient outcomes.”

Sylfirm X by BENEV
JongJu Na, MD, PhD, an inventor in South Korea, yearned for a device that targeted pigmentation and anti-aging for all skin types, with reduced treatment time, improved comfort, and little to no downtime. The result was Sylfirm, featuring traditional adjustable-depth CW and Dr. Na’s patented PW delivery modes to treat both the face and body. The safety and versatility of this technology, and the subsequent outcomes, are supported by 26 publications* and 16+ patents.

“My patient demographic is anywhere from 21 to 70 years of age, and the needs of each are different,” said Renato Saltz, MD, director of Saltz Plastic Surgery & Saltz Spa Vitoria in Salt Lake & Park City, Utah. “The versatility of Sylfirm X Ultimate Edition improves my ability to treat a wider range of patients,” he exclaimed.

Undoubtedly the most profound advantage of Sylfirm X Ultimate Edition is the ‘Na Effect,’ named for its inventor and detailed in a 2015 study of tissue reactions to bipolar RF microneedling. Na et al. performed in vivo treatment of micro pig skin and ex vivo treatment of bovine liver tissue with RF microneedling, varying needle insertion depth and conduction time in both cases. Tissue reactions in and around thermally induced coagulation columns were carefully noted via histologic examination.2

Overall, Na et al. found that by using uninsulated needles and 2-second conduction times with minimal surface penetration, individual regions of thermal coagulation around each pointed needle tip converged with nearby coagulation zones. This rapidly produced a pronounced effect at the surface, limiting depth of coagulation and reducing treatment time to under 20 minutes. Damage, but not destruction, of vascular tissue in the treatment area was noted, while preserving the surrounding skin.2

Sylfirm X penetrates from 300 microns to 4 mm treating all skin layers. This allows the ability to address the epidermis, basal membranes, and papillary dermis, thus enabling treatment of superficial blemishes to deeper structures, promoting color correction and collagen remodeling while utilizing a single tip.

“I did not need another piece of expensive equipment in my office, I needed a platform that could allow me to offer a wider range of treatments, was simple enough for my staff to use, and provides replicable treatment outcomes,” said Chad Deal, MD, director of Southern Surgical Arts in Chattanooga, Tenn.

“The platform had to be safe and effective on Fitzpatrick skin types I-VI and have clinical support,” Dr. Deal continued. “For years I looked, but I had no luck until I found Sylfirm X. This device changed my practice, and my patient satisfaction is unparalleled. I can now address both pigmentation and skin laxity, improve skin health, and combine the treatments with PDO threads or a facial. My patients now come in consistently and see results in less time. Patients trust me with their treatment and it is my responsibility to offer them the best option available.”

According to Raminder Saluja, MD, director of Saluja Cosmetic and Laser Center (Huntersville, N.C.), “the results my patients and I are seeing even after the first treatment is incredible. My patients say this is by far the most comfortable treatment I offer, and this translates into a steady stream of patient bookings and referrals.”

For pigmentation and vascularity, Sylfirm X’s unique PW modes provide softer, more selective coagulation in the basal membrane and vascular structures, targeting abnormal small blood vessels while leaving surrounding tissue virtually unaffected. Recovery is rapid, and the efficacy and mechanism of action are currently confirmed by ten studies.*

“This is my go-to device,” said Jessica Christy, PA-C, owner of Beauty Culture MedSpa in Bloomfield Hills, Mich. “Sylfirm X has replaced all other microneedling and RF devices in my practice. This device can be used from the scalp all the way down to the toes. For what this device does and for what it replaces, the price is a steal.”

Exosomes by BENEV
BENEV exosomes are designed and developed in tandem by BENEV Company Inc. – a premier skin solution brand for medical professionals, and a U.S. FDA-registered manufacturing facility – and ExoCoBio (Seoul, South Korea), the fourth largest exosome research company in the world.

BENEV Exosome Regenerative Complex+ (skin and scalp) and Exosome Regenerative Complex (skin) is taking over the post treatment aesthetic market. Favored by thousands of medical practitioners throughout North America, these clinically proven, dual action complexes absorb quickly into the skin delivering the concentrated power of stem cell derived lyophilized exosomes, potent growth factors, peptides, coenzymes, minerals, hyaluronic acid, amino acids and vitamins.

Exosome Regenerative Complex and BENEV Exosome Regenerative Complex+, powered by Exo-SCRTTM, are post-care topical use solutions. The isolated and purified exosomes in both products have been accepted by the Personal Care Products Council (PCPC), and International Cosmetic Ingredient Nomenclature Committee (INC), and the title ‘Human Adipose Stromal Cell Exosomes’ was given and published in the International Cosmetic Ingredient Dictionary and Handbook (INCI). At this time, there are no other cosmetic INCI name(s) assigned to exosome products in the market.

To fully understand this product and its capabilities, one must first understand the origin of exosomes and the role they play. Randy Miller, MD, medical director of Miller Plastic
Surgery (Miami, Fla.) explained, exosomes are responsible for cell-to-cell communication and are thus vital to the regenerative process at the cellular level much more directly than seen with Platelet-Rich Plasma (PRP) and/or growth factor concentrations. These nanoparticles are naturally able to penetrate the stratum corneum, making them ideal for topical application, as well as adjunctive use with other therapies. Overall, this promotes the production of healthier, more youthful tissue.

“Exosomes are definitely the future of aesthetic and general medicine,” Dr. Miller continued. “Exosomes derived from stem cells activate the mechanisms we are looking to harness, and adipose tissue is the richest source available. With BENEV Exosome Regenerative Complex+ we see a 180% increase in fibroblast production and around a 300% increase in nucleogenesis.* This is significantly beyond what one could achieve using something like PRP. Additionally, these nanoparticles are clinically known for reducing inflammation, which translates to a more comfortable treatment and reduced treatment downtime, which is something every medical provider should integrate into all their treatments. There is just no reason why someone would not want to include them in their practice. The clinical science is unmatched and BENEV has taken away the issue of cost.”

While TJ Tsay, MD, owner and director of Ageless MD (Tustin, Calif.), confirmed that PRP will remain useful for quite some time, he shared how powerful exosomes are as they stimulate the regenerative cascade while minimizing inflammation. “BENEV Exosome Regenerative Complex+ delivers a variety of more than 50 active components, including growth factors, directly to cells in the treatment area using the very mechanisms of natural intercellular communication,” he explained. “This makes BENEV Exosome Regenerative Complex+ the ultimate addition to the regenerative aesthetics armamentarium by itself or to supplement any therapy. The potential applications across the entire field of medicine are staggering.”

According to J.D. McCoy, ND, medical director of Contour Medical (Gilbert, Ariz.), BENEV Exosome Regenerative Complex+ is also unique in that it is the only product stable enough for reconstitution at the time of use. For him this product is a go-to for augmenting hair restoration therapy. “The product is powerful and aggressive but still natural with clinically proven safety and efficacy, and its utility is impressive. You can use it as a stand-alone therapy, but BENEV Exosome Regenerative Complex+ powered by ExoSCRT excels when added to your existing treatment protocols to improve healing, reduce recovery time, and maximize the onset of results. It is also relatively affordable. Anyone who doesn’t adopt this is missing out.”

These products are not drugs. They are not intended to prevent, treat or cure diseases or medical conditions. They are not intended to be injected or delivered intravenously. The patented, paraben-free, steroid-free and hypoallergenic technologies and ingredients in both of these products are clinically proven to rejuvenate and regenerate the skin and scalp.*

MIRACU PDO Threads by BENEV
Polydioxanone (PDO) absorbable threads have taken hold in aesthetic medicine because they provide a minimally invasive alternative to tissue lifting while stimulating a regenerative reaction in the body, promoting neocollagenesis and acting as a biodegradable scaffold at the same time. “PDO threads have been around for a while but not as widely adopted as other technologies for a variety of reasons,” according to Scott Callahan, PA-C owner and director of Dolce Vida Medical Spa with locations in Trumbull, Greenwich and Westport, Conn.

“There were too many hassles associated with them,” Mr. Callahan emphasized. “Threads were not pre-loaded in their cannulas and I had to purchase from re-packagers rather than the manufacturers, with not enough of a variety of threads for me to address patient concerns. In addition, threads were cut rather than molded, and thinner threads did not provide the level of correction that I was comfortable with, often breaking in the packaging, rendering them useless. On top of this, patients reported that thicker threads could be felt, which is another problem of its own.”

MIRACU PDO Threads manufactured by DongBang Medical Co. Ltd. (Sungnam, Korea), the largest PDO and cannula manufacturer in the world, are sold through BENEV Company Inc. in North America, Canada and Mexico. Innovative MIRACU PDO threads are FDA cleared for soft tissue approximation where an absorbable suture is appropriate. These competitively priced specialty threads offer tensile strength 10 to 20 times greater than other PDO threads, with sterile individual packaging.

“They received FDA approval for both threads and insertion cannulas before launch, which appealed to me considerably and is unique as far as I know,” Mr. Callahan added. “This reflects the care and extensive research and development that went into MIRACU PDO Threads.”

Forte Fix threads are the latest addition to the MIRACU line of specialty PDO threads. Forte Fix provides superior traction and holding through 4D Multi Bi-Directional Blocking Technology and Enhanced Dual Action Technology. Deep butterfly-shaped cogs for horizontal fixation, and round shaped hook cogs (upward & downward), offer improved anchoring and vertical fixation.

Together with Forte Basic, Elasty Line and Elasty Meshfill, these specialty threads truly highlight the brand’s innovative approach. The full line includes an extensive variety of thread types, sizes, gauges and cannula tips translating into better results, fewer insertions, less overall product, treatment time and cost.

As the sole cone anchor thread on the market, Elasty Line is manufactured via unique methods to provide a high polymer double coating. The exclusive FCV cannula tip allows for the PDO cone anchor tip to sit on the cannula and offer more accurate placement with superior fixation. While traditional molded cogs have shallower hooks and comparatively weak fixation, the shape of each Elasty Line deep cog hook is designed for strong fixation and powerful lifting. “This is the strongest tensile strength available and it is a go-to for me in my practice,” reported Gideon Kwok, DO, co-owner and medical director of more than eight Skin Perfect clinics around Southern California.

Jeffrey Rosenthal, MD, owner and director of Art of Plastic Surgery in Fairfield, Conn., uses Elasty Line threads, Elasty Meshfill, Forte Basic and Forte Fix to fill a particular niche in his practice. “They offer outstanding, customizable correction when used for treatments on the face and body,” he expressed. “I specifically use the Elasty Line thread and Forte thread as they provide the lift my patients are looking for.”

Unique in being a 16-line mesh thread, Elasty Meshfill delivers maximum volume via the 2-in- 1 technology created from 16 mono threads woven together to create a hollow cylindrical mesh thread ideal for volumization and rejuvenation. This means Experts Praise the Regenerative Aesthetic Solutions Transforming Their Practicesa stronger, softer thread with greater elasticity than alternative mesh threads. Jason Emer, MD, owner and director of Jason Emer MD in West Hollywood, Calif., explained that Elasty Meshfill was designed to be more scaffold-like than traditional threads to provide better support. “After insertion between the submuscular aponeurotic system (SMAS) and the dermis, it takes about two weeks for soft tissue to fill in the gaps created by the scaffold provided by Elasty Meshfill threads.

“There is a synergistic effect when combined with fillers,” Dr. Emer continued. “Once implanted, the 16-line mono-mesh thread reduces diffusion and migration of the filler material. This is regenerative aesthetics. The PDO threads stimulate collagen production in addition to any filler-related correction so when using this kind of protocol, we use less filler product but get up to 40% improvement in longevity of the outcomes. Less filler product means less cost, and we get a better, longer-lasting result. You can augment the treatment further by adding PRP, and it is also a standout alternative to implanting larger volumes of filler in hard-to-treat locations.”

With three leading-edge products and continued development, BENEV and DBM is paving the way for others in the field of regenerative aesthetics.
